We do not accept State Insurances, such as Medicaid, Meridian , etc.

Therapy with Insurance: For clients with an accepted and active medical insurance coverage, client's portion of payment, e.g., copays and coinsurances, is collected at the time of service. Our office does our best to provide the most accurate cost breakdown/estimate based on insurance information available to us. We strongly encourage clients to verify health insurance coverage, e.g., deductibles, copays, coinsurance, etc. prior to the initial appointment. 

Therapy with a Sliding Fee Scale : for clients who have insurance that the clinician does not accept and/or clients that choose not to use their insurances. The sliding fee-scale is based on client's household gross annual income and the total number of dependents in the household. A proof of income, e.g., the most recent pay stub and W-2s, may be required at the time of intake. The sliding fee scales vary slightly among the therapists. Please call the office to verify. 

Standard Costs for Therapy:  for clients with no insurances and/or do not qualify for a sliding fee scale. $300 Initial/Intake Session; $250 Individual Therapy Session; $275 Family Session. 

Psychological Testing With Accepted Insurance: We accept BCBS Illinois (not Choice) PPO, Wellmark, and UHC/UBH/Optum for psychological evaluations, e.g., ADHD, autism, learning disabilities, developmental disorders, behavior disorders, personality disorder, full psychological evaluation, and neuropsychological evaluation. 

Psychological Testing Without Accepted Insurance or With No Insurance: Please call the office to inquire the current flat rate for a psychological evaluation for clinical diagnoses purposes. See below for forensic evaluation rates. 

Forensic Evaluations: Parenting time, Parenting Capacity, Child Custody, Parental Alienation, Supreme Court Rule 215, FOID, Fitness for Duty, Group Home Placement, Adoption, Surrogacy, Immigration, etc. Please contact our office for individual rate of each type of forensic evaluation. 

Payment Methods: We accept Credit/Debit cards (MasterCard, Visa, Discover, American Express), cash, and check. 

Late Cancellation Fees: Late cancellation fee policies are discretionary decisions made by each individual contractual clinician, and do not reflect or represent other clinicians in this group or the group practice policy in this specific area. Please discuss it with your rendering provider regarding his/her late cancellation policy.
