Finances and insurance

 Insurance participation and payment responsibilities vary depending on the service requested and, in some cases, by clinician. We encourage all clients to verify their insurance benefits, including deductibles, copays, and coinsurance amounts, before the initial appointment. We do not accept state insurance plans, such as Medicaid or Meridian.

 For clients with accepted and active health insurance coverage, the client’s portion of payment, including copays, coinsurance, and deductible amounts, is due at the time of service. Our office does its best to provide the most accurate estimate based on the insurance information available to us. Clients remain responsible for verifying their own coverage and benefits directly with their insurance carrier. 

 Some clinicians may offer a reduced sliding-fee rate for clients whose insurance is not accepted or who choose not to use insurance. Sliding-fee eligibility is generally based on household gross annual income and the number of dependents in the household. Proof of income, such as a recent pay stub or W-2, may be required at intake. Sliding-fee policies vary by clinician. Please contact our office if you would like to inquire about a particular clinician’s current sliding-fee options. 

 We accept most BCBS policies, Wellmark, UHC / UBH / Optum and Aetna Commercial plans for medically necessary psychological evaluations, including ADHD evaluations, autism evaluations, learning disability evaluations, developmental evaluations, behavioral evaluations, personality evaluations, full psychological evaluations, and some neuropsychological evaluations.

 If we do not accept your insurance for psychological testing, or if you do not have insurance coverage for testing, please contact our office to inquire about the current flat-rate self-pay fee for a clinical psychological evaluation. 

 Please contact our office directly regarding the rate for a particular forensic or specialty evaluation. 

We accept credit cards, debit cards, cash, and check. Accepted cards include Visa, MasterCard, Discover, and American Express.  

Late cancellation fee policies are set individually by each contractual clinician and may vary. These policies do not necessarily reflect a uniform group practice policy in this area. Please discuss your clinician’s late cancellation policy directly with your rendering provider.
